The space between the base of the nose and the top of the upper lip is known as the philtrum. As we age, this area naturally elongates due to a loss of skin elasticity and the effects of gravity, which can cause the upper lip to thin and turn inward, hiding the teeth. In younger patients, a long philtrum may be a genetic trait that affects the overall balance of the face. A lip lift provides a structural solution to these concerns, restoring a more energetic and youthful mid-facial appearance.
What is a Lip Lift?
A lip lift is a precise surgical intervention that involves removing a small, "bullhorn-shaped" strip of skin from directly beneath the nostrils. By removing this excess tissue, the surgeon elevates the position of the upper lip. This elevation increases the vertical height of the pink lip tissue (the vermilion), enhances the "Cupid’s bow," and restores a youthful "tooth show" when the mouth is slightly open or during a smile.
While there are various techniques, the most common and effective is the Subnasal Bullhorn Lip Lift. The incision is carefully hidden in the natural shadow and contours of the base of the nose. Other variations, such as the Corner Lip Lift, focus specifically on the outer edges of the mouth to correct a "down-turned" or sad expression.
Who is a lip lift for?
A lip lift is an ideal procedure for individuals who find that dermal fillers make their upper lip look "heavy" or "duck-like" rather than lifted. You may be a candidate for this surgery if:
- You have a long philtrum (the space between the nose and lip).
- Your upper lip appears thin or lacks a defined Cupid's bow.
- Your upper lip hides your teeth when you speak or smile.
- You wish to achieve a more permanent result than fillers can provide.
- You feel your upper lip has flattened and elongated due to the natural aging process.
Before the operation
The process begins with a detailed facial analysis. Your surgeon will measure the length of your philtrum and assess how much dental show occurs at rest. A typical ideal distance is between 12mm and 15mm, though this varies based on individual facial height. You will discuss your goals, and your surgeon will demonstrate the expected lift by gently elevating the lip skin during the examination.
Preoperative instructions include a strict requirement to stop smoking for at least four weeks, as nicotine significantly impairs the healing of the delicate skin beneath the nose. You must also avoid blood-thinning medications and supplements to minimize the risk of bruising and ensure the finest possible scar.
During the operation
A lip lift is typically performed under local anesthesia with sedation or general anesthesia and takes about 45 to 60 minutes. The surgeon marks the "bullhorn" shape precisely at the base of the nose, ensuring the incision follows the natural curves of the nostrils.
Once the small strip of skin is removed, the underlying tissues are lifted and secured with deep, tension-reducing sutures. This internal support is crucial to prevent the scar from stretching over time. The skin is then meticulously closed with very fine sutures. A small amount of antibiotic ointment and a light dressing may be applied.
After the operation and recovery
In the first week, swelling is common and can make the upper lip appear unnaturally high; this is temporary and subsides as the tissues relax. You may experience some tightness and minor discomfort, which is managed with standard pain relief. It is important to limit exaggerated facial expressions, such as wide smiling or heavy chewing, for the first two weeks to protect the incision.
Sutures are typically removed after 5 to 7 days. Most patients return to work and social activities within 10 days, although the incision line may remain slightly pink for several weeks. This can be easily camouflaged with concealer once the sutures are out. The final result—a more defined, youthful, and proportional lip—becomes fully apparent after 3 months as the internal tissues settle.
Is a lip lift better than lip fillers?
Fillers are excellent for adding temporary volume, but they cannot shorten a long philtrum. In fact, adding filler to a long upper lip can often make it look heavier and more elongated. A lip lift addresses the structural length of the lip, providing a permanent change in shape and position that fillers simply cannot achieve.
Where will the scar be?
The incision is placed in the "nasal sill," the natural crease where the base of the nose meets the upper lip. When performed by a skilled surgeon, the resulting scar is tucked into the shadows of the nostrils and becomes virtually imperceptible once it has fully matured.
Will my smile look different?
Yes, but in a positive way. A lip lift is specifically designed to improve "incisal show," meaning more of your upper teeth will be visible when you smile. This is a hallmark of a youthful facial structure. Your surgeon will ensure the lift is conservative enough to maintain a natural, functional smile.
Can I have a lip lift if I have a mustache or facial hair?
Yes. For male patients, the incision is still placed at the base of the nose. While the surgery does not significantly affect hair growth, we recommend being clean-shaven for the procedure to allow for the most precise incision and closure.
